— MADISON — Mildred Milkovits, age 90, a resident of Madison, Ohio, formerly of Bath, Pa., passed away Tuesday, May 13, 2008, at Rae-Ann Geneva Skilled Nursing & Rehabilitation Center, Geneva.
Born Oct. 7, 1917, in New York City, N.Y., she was a daughter of the late Marie (Hllavach) and Joseph Polivka.
Survivors include a son, Stephen (Suzanne) Burk of Madison; two grandsons, Stephen M. Burk and Scott R. Burk, both of Geneva; two great-grandchildren, Sarah and Allan; a sister, Olga Bickford of Ephrata, Pa.; and companion, John P. Milkovits of Bath, Pa.
She was preceded in death by her parents; a sister, Bernardin Filchner; and brother, George Polivka.
Funeral Service will be 11 a.m. Friday, May 16, 2008, at The Behm Family Funeral Home, 26 River St., Madison, Ohio. Final resting place will be in Fairview Cemetery, Madison.
Friends will be received from 6 to 8 p.m. Thursday, May 15th, at the Funeral Home.
